Patron Card Web Application

If you would like to apply for a MidPointe Library System card, please fill out and submit the form below. Submissions are welcome any time, but applications are only processed Monday-Friday, 8:00 AM to 5:00 PM. Upon processing, applicants will receive a digital card number and PIN via email that will allow you to use MidPointe’s digital collection and place holds on physical items. A physical library card will be mailed to your home address. When you visit the Library, please be prepared to show proof of ID.

I understand that I am responsible for all fines and fees and any ítems checked out on my card prior to being reported lost or stolen.
